Your application was acted upon by the Planning Department of the City of Huntington Beach
on May 13, 2004, and your request was Conditionall A roved. Included in this letter are the
Conditions of Approval for this application. You must submit this notice of action letter to the
Building Department upon request for Certificate of Occupancy and/or Building Permits.
Under the provisions of the Huntington Beach Zoning and Subdivision Ordinance, the action
taken by the Planning Department becomes final at the expiration of the ten-day appeal period.
A person desiring to appeal the decision shall file a written notice of appeal to the Secretary of
the Planning Commission within ten calendar days of the date of the Planning Department's ac-
tion. The notice of appeal shall include the name and address of the appellant, the decision being

appealed, and the grounds for the appeal. A filing fee shall also accompany the notice of appeal.
The appeal fee is $485.

CONDITIONS OF APPROVAL - ADMINISTRATIVE PERMIT NO. 04-06
1. The floor plans received and dated April 28, 2004, shall be the conceptually approved layout.
2. The maximum number of participants/customers shall not, at any time, exceed the number of
parking spaces allocated to the suite, based on a parking standard of one parking space per
200 square-feet of floor area. Accordingly, the maximum number of fitness center partici-
pants in the proposed 1,400 sq. ft. studio shall be limited to seven (not including employees).
3. The training area shall not exceed 75 percent of the total floor area of the personal enrich-
ment building area, or 1,050 square-feet.
4. The applicant and/or applicant's representative shall be responsible for ensuring the accuracy
of all plans and information submitted to the City for review and approval.
INFORMATION ON SPECIFIC CODE RE UIREMENTS:
1. Administrative Permit No. 04-06 shall not become effective until the ten-calendar day ap-
peal period has elapsed.
2. Administrative Permit No. 04-06 shall become null and void unless exercised within one
year of the date of final approval or such extension of time as may be granted by the Direc-
tor pursuant to a written request submitted to the Planning Department a minimum 30 days
prior to the expiration date.
3. The Planning Department reserves the right to revoke Administrative Permit No. 04-06,
pursuant to a public hearing for revocation, if any violation of these conditions or the Hunt-
ington Beach Zoning and Subdivision Ordinance or Municipal Code occurs.
4. The use shall comply with all applicable provisions of the Municipal Code, Building De-
partment and Fire Department as well as applicable local, State and Federal Fire Codes, Or-
dinances and standards, except as noted herein.
5. A Certificate of Occupancy must be approved by the Planning Department and issued by
the Building and Safety Department prior to occupying the building.
