In May, 2005, our land use entitlement agent, Scott Longhurst from Trillium Telecom Services met with Planning staff, and, at that time, your Director determined that the project could be approved administratively because of the stealth design and because it was a co-location facility, and indicated that the project should go straight to the Building Department for plan check and building permits (see attached memo). Because of Hurricane Katrina, Verizon Wireless' build funds were diverted from the West Coast Region to rebuild damaged cell sites and construct new sites in the Gulf Coast areas for close to two years. Verizon now wishes to move forward to build the "Adams Beach" site. Based upon information provided by Frank Carter, it is my understanding that the FIB Planning staff is now requiring that Verizon submit a "Wireless Permit Application", which is a new requirement that the City has instituted within the last year with the adoption of your new Wireless Ordinance. The Planning Consortium will move forward and prepare this application package for City staff review. I would like to request relief from Section 5.00 of the submittal requirements related to the required I of 2 1/30/2008 12:24 PM The Planning Consortium, Inc. Wireless PennitAppiication Verizon Wireless Communication FacilityAdams Beach" 19671 Beach Boulevard January 30, 2008 Page 2 The radio equipment area is located adjacent to the building's penthouse on the roof within a screened area. The equipment area measures approximately 35 feet long, 13 feet wide, and does not exceed the height of the adjacent existing stairwell penthouse. This area will contain four radio equipment cabinets, a step down transformer and power and telephone connection cabinets. The proposed screened equipment enclosure will be painted to match the black color of the existing penthouse. Attached are detailed plans (three reduced sets) that show the proposed Verizon Wireless improvements. Wireless A lication Form Attached is the completed City of Huntington Beach "Wireless Application Form" with the required attachments. Section 1.00: The applicant is Verizon Wireless. The Planning Consortium is Verizon Wireless' agent, who will be processing the City required discretionary permits. Attached is a letter from Verizon Wireless that allows The Planning Consortium to act as their agent for this application (Attachment 2.08). The property owner is "Beach Metro, LLC"; the Managing Member is Scott C. Looney. Section 2.00: Attached is a "Letter of Authorization" from the Beach Metro, LCC that authorizes Verizon Wireless to proceed with the application process. This Letter of Authorization is included as Attachment 2.09. Section 3.00: This project is primarily to provide new radio frequency coverage in an area not already served by existing radio frequency coverage by Verizon Wireless. Section 4.00: Enclosed are the required "Radio Frequency Coverage Maps". Attachment 6.02 shows the current RF coverage provided by existing VzW cell sites and the "hole in coverage" where the proposed site is located. Attachment 6.03 shows the RF coverage provided by only the proposed "Adams Beach" project. Attachment 6.04 shows the coverage from the surrounding network and the proposed project, should the project be approved. Section 5.00: Attached are Project Photographs and a Photo Simulation. Attached is one "before" and "after" photo simulations that show the project location from the Beach Boulevard public right-of way. Also included is a Key Map of the view for the photo simulation. This aerial photo key map also shows adjacent and surrounding land uses. Section 6.00: Candidate sites are limited in this area of Huntington Beach. This specific location was selected because of the height of the existing building, because it is a co-location with an existing wireless facility, because there is room for the equipment on the roof, and because there is a landlord willing to allow a second wireless facility at this location. Because of these conditions, and because there are no other sites in Verizon's "search ring" area, no other alternative locations were pursued. There are no other alternative locations within %4 to V2 mile of the proposed project that is suitable for a wireless communication facility. This information is included as Attachments 8.04 and 8.07, and attached to the Wireless Permit Application Form. Section 8.00: Form Certification This section certifies that on the behalf of Verizon Wireless and Beach Metro, LLC, the answers provided within the Wireless Permit Application Form are true and complete to the best of The Planning Consortium's knowledge.
