A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF HUNTINGTON BEACH
AUTHORIZING THE MAYOR AND CITY CLERK TO EXECUTE PROGRAM
SUPPLEMENT NOS. MO 12, MO13, MO 14 AND MO 15 TO AGREEMENT NO. 12-5181 FOR THE
BROOKHURST STREET, EDINGER AVENUE, SPRINGDALE STREET AND MAGNOLIA
STREET REHABILITATION CONSTRUCTION IMPROVEMENTS
WHEREAS, on June 16, 1997, the City entered into an agreement with the State of California
for the Federal Aid Program, identified as Agreement 12-5181; and
Program Supplement Nos. MO 12, M013, MO 14 and M015 to the above mentioned Master
Agreement require arterial highway rehabilitation construction improvements; and
The estimated project cost for each agreement is as follows: M012 (Brookhurst Street)
$1,009,434, M013 (Edinger Avenue) $467,694, M014 (Springdale Street) $599,935 and M015
(Magnolia Street) $836,791. The sum of estimated project cost of all four agreements is 2,913,854; and
From the estimated project cost, the following dollar amounts will be appropriated from Federal
and State funds to their respective project as shown on Attachments A hereto: MO 12 (Brookhurst
Street) $400,000, MO13 (Edinger Avenue) $233,847, MO 14 (Springdale Street) $269,895 and MO 15
(Magnolia Street) $237,659_ The sum of Federal and State funds allocated for all four projects is
$1,141,401; and
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Huntington Beach
that the Mayor and City Clerk are authorized and directed to execute the necessary documents to
implement Program Supplement Nos. M012, M013, MO 14 and MO 15 to Master Agreement No. 12-5181
for the Brookhurst Street, Edinger Avenue, Springdale Street and Magnolia Street Rehabilitation
Improvements.
PASSED AND ADOPTED by the City Council of the City of Huntington Beach at a regular
meeting thereof held on the 20th day of August , 2001.

SPECIAL COVENANTS OR REMARKS
1. The ADMINISTERING AGENCY will reimburse the State for their share
of costs for work requested to be performed by the State.
2 . The ADMINISTERING AGENCY will advertise, award and administer
this project in accordance with the current Local Assistance
Procedures Manual.
3 . All maintenance, involving the physical condition and the
operation of the improvements, referred to in Article III
MAINTENANCE of the aforementioned Master Agreement will be the
responsibility of the ADMINISTERING AGENCY and shall be performed
at regular intervals or as required for efficient operation of
the completed improvements.
4 . The ADMINISTERING AGENCY is required to have an audit in
accordance with the Single Audit Act and OMB A-133, if it
receives $300, 000 or more in federal funds in a single fiscal
year:4 The -federal funds received under this project are a part
• of the Catalogue of Federal Domestic Assistance (CFDA) 20.205,
Highway Planning and Research. OMB A--133 superceded OMB A-128 in
1996. Reference to OMB A-128 in Master Agreement (if any) is
superceded by this. covenant.
5 . The ADMINISTERING AGENCY agrees that payment of Federal funds
will be. limited to the amounts approved by the Federal Highway
Administration (FHWA) in the Federal-Aid Project
Authorization/Agreement or Amendment/Modification (E-76) and
accepts any increases in ADMINISTERING AGENCY funds as shown on
the Finance or Bid letter or its modification as prepared by the
Division of Local Assistance, Office of Project Implementation.
6. Award information shall be submitted by the ADMINISTERING AGENCY
to the District Local Assistance Engineer immediately after the
award. Failure to do so will cause delay in processing the
..invoices for the construction phase. Please refer to Section
15.7 "Award Package" of the Local Assistance Procedures Manual.
