Hopkins, PE, Director of Public Works SUBJECT: Approve and authorize execution of four Professional Services Contracts each in an amount not to exceed $1,000,000 with CivilSource, Inc., Dudek, Inc., Harris and Associates, Inc. and Psomas, Inc. for On -Call Construction Management Services Statement of Issue: The Public Works Department requires professional Construction Management services on an on - call or as -needed basis to support staff for the construction of the capital improvement projects requiring material testing services. Financial Impact: The City's FY 2015/16 Capital Improvement Program (CIP) Budget was adopted on September 8, 2015 and totals $27.4 million (All Funds) for approved projects citywide. Each approved CIP project has a budget containing sufficient funding for design, environmental, engineering, construction, project management and other required costs. The four (4) recommended $1 million construction management contracts will be funded over 3 years within each project's individual CIP budget, as needed. As such, no additional funding is required by this action. A) Approve and authorize the Mayor and City Clerk to execute a 3-year, not -to -exceed $1,000,000 "Professional Services Contract Between the City of Huntington Beach and CivilSource, Inc. for As - Needed Construction Management Services;" and, B) Approve and authorize the Mayor and City Clerk to execute a 3-year, not -to -exceed $1,000,000 "Professional Services Contract Between the City of Huntington Beach and Dudek for As -Needed Construction Management Services;" and, LL,! Alternative Action(s): Do not authorize the contracts and direct staff with an alternate action. Budgeted Capital Projects could be delayed until new contracts are approved. Item 10. - 1 FIB _J36- Dept. ID PW 16-038 Page 2 of 3 Meeting Date: 8/1/2016 Analysis: On May 5, 2016, the City advertised a Request for Qualifications ("RFQ") for Construction Management Services. Proposals were requested and submitted in compliance with Chapter 3.03 of the Huntington Beach Municipal Code. Twenty -Three (23) proposals were received for construction management services. In accordance with qualifications based selected procurement per the Federal "Brooks Act," State SB 419 and the City of Huntington Beach Municipal Code Section 3.03 "Professional Services" the Public Works Department established a review board consisting of the following subject matter experts from the Public Works Department evaluated and ranked the proposals: Contract Administrator, Senior Civil Engineer, and Civil Engineering Assistant. The ranking criteria included: • RFP Clarity • Understanding & Methodology • Firm Qualifications • Staff Qualifications & Experience • References The selection process included a review of the written proposals, followed by an in depth interview with the top ranked six vendors. Summary evaluation sheets for the six most qualified firms are shown as Attachment 2. Each firm stated their construction management category of expertise with their proposal, and proposals were then evaluated and ranked by three (3) review board members who were established by the Public Works Department. After conducting the entire evaluation process, including interviews and reference checks, the committee determined that Harris, Dudek, P s o m a s, and C i v i I S o u r c e are the most qualified firms to meet the needs of the City and most qualified to perform work on federally funded projects as well as providing competitive proposals for future projects. The four (4) contracts recommended in this request for council action (RCA) are construction management services for construction of the capital improvement projects related to all types of CIP projects. These services have historically been utilized to supplement City staff to meet annual CIP goals, as an extension of staff and/or to provide required expertise for unique projects. It is anticipated that these services will be required based on past years. Each project is evaluated on a case by case basis to determine if these as -needed contract services are necessary. The City's Capital Improvement Program (CIP) is approximately $27.4 million in Fiscal Year 2015/16, and exceeds $35 million with continuing of funds for projects approved from previous years. Typical construction support cost from use of consultants on a CIP project can vary between ten (10) to twenty (20) percent of total costs for a CIP project. Therefore, it is practical to conservatively estimate minimum annual consultant costs would be around $1.33 million to the City for total CIP projects or approximately $4 million over the next three (3) years. Construction management services are typically required for all types of CIP projects, such as for facilities, streets, water, or sewer. Therefore, four (4) consultants were selected to meet projected workload over the next three years. Sum of these contracts is $4 million over the term of the contract or an annual average of $1 million. Public Works Commission Action: Not required. FOR AS -NEEDED CONSTRUCTION MANAGEMENT SERVICES THIS AGREEMENT ('Agreement") is made and entered into by and between the City of Huntington Beach, a municipal corporation of the State of California, hereinafter referred to as "CITY," and CiviiSource, Inc, a corporation hereinafter referred to as "CONSULTANT," YHEREA-S—C—ITS—desires to—engageAhe--services-ofa consultant t:o—provide as=needed- ----- construction management services- and Pursuant to documentation on file in the office of the City Clerk, the provisions of the Huntington Beach Municipal Code, Chapter 3.03, relating to procurement of professional service contracts have been complied with; and CONSULTANT has been selected to perform tlieseservices, NOW, THEREFORE, it is agreed by CITY and CONSULTANT as follows: 1. SCOPE OF SERVICES CONSULTANT shall provide all services as described in Exhibit "A," which is attached hereto and incorporated into this Agreement by this reference. These services shall sometimes hereinafter be referred to as the "PROJECT." CONSULTANT hereby designates Amy Amirani who shall represent it and be its sole contact and agent in all consultations with CITY during the perfonnance of this Agreement. 2. All tasks specified in Exhibit "A" shall be completed no later than three years from the Commencement Date. COMPENSATION In consideration of the performance of the services described herein, CITY agrees to pay CONSULTANT on a time and materials basis at the rates specified in Exhibit "B," which is attached hereto and incorporated by reference into this Agreement, a fee, including all costs and expenses, not to exceed one million Dollars ($1,000,000). 5. EXTRA WORK In the event CITY requires additional services not included in Exhibit "A" or changes in the scope of services described in Exhibit "A," CONSULTANT will undertake such work only after receiving written authorization from CITY. Additional comperrsation for such extra work shall be allowed only if the prior written approval of CITY is obtained. 6. DISPOSITION OF PLANS, ESTIMATES AND OTHER DOCUMENTS CONSULTANT agrees that title to all materials prepared hereunder, including, without limitation, all original drawings, designs, reports, both. field. and office notices, calculations, computer code, language, data or programs, maps, memoranda, letters and other documents, shall belong to CITY, and CONSULTANT shall turn these materials over to CITY upon expiration or termination of this Agreement or upon PROJECT completion, whichever shall occur first. HOLD HARMLESS CONSULTANT hereby agrees to protect, defend, indemnify and hold harmless CITY, its officers, elected or appointed. officials, employees, agents and volunteers from and against any and all claims, damages, losses, expenses, judgments, demands and defense costs (including, without limitation, costs and fees of litigation of every nature or liability of any kind or nature arising out of or in connection with CONSULTANT's (or CONSULTANT's subcontractors, if any) negligent (oralleged negligent) performance of this Agreement or its failure to comply with any of its obligations contained in this Agreement by CONSULTANT, its officers, agents or employees except such loss or damage which• was caused by the sole negligence or willful misconduct of CITY. CONSULTANT will conduct all defense at its sole cost and expense and CITY shall ---------------- approve selection of CONSULTANT's counsel. This indemnity shall apply to all claims and liability regardless of whether any insurance policies are applicable. The policy limits do not act as limitation upon the amount of indemnification to be provided by CONSULTANT. 9. PROFESSIONAL LIABILITY INSURANCE CONSULTANT shall obtain and furnish to CITY a professional liability insurance policy covering the work performed by it hereunder. This policy shall provide coverage for CONSULTANT's professional liability in an amount not less than One Million Dollars agree/ surfnet/professionai svcs mayor 3 of 11 10/12 ($1,000,000.00) per occurrence and in the aggregate. The above -mentioned insurance shall. not contain a self -insured retention without the express written consent of CITY; however an insurance policy "deductible" of Ten Thousand Dollars ($10,000.00) or less is permitted. A claims -made policy shall be acceptable if the policy further provides that: A. The policy retroactive date coincides with or precedes the initiation of the scope of work (including subsequent policies purchased as renewals or ru B. CONSULTANT shall notify CITY of circumstances or incidents that might give rise to future claims. CONSULTANT will make every effort to maintain similar insurance during the required extended period of coverage following PROJECT completion. If insurance is terminated for any reason, CONSULTANT agrees to purchase an extended reporting provision of at least two (2) years to report claims arising from work performed in connection with this Agreement. If CONSULTANT fails or refuses to produce or ivai stain the insurance required by this section or fails or refuses to furnish the CITY with required proof that insurance has been procured and is in force and paid for, the CITY shall have the right, at the CITY's election, to forthwith terminate this Agreement. Such termination shall not effect Consultant's right to be paid for its time and materials expended prior to notification of termination. CONSULTANT waives the right to receive compensation and agrees to indemnify the CITY for any work performed prior to _ ..... ---- --- --- - - -- -- - approval of insurance by the CITY. 10. The requirement for carrying the foregoing insurance coverage shall not derogate from CONSULTANT's defense, hold harmless and indemnification obligations as set forth in this Agreement. CITY or its representative shall at all times have the right to demand the original or a __copy of the policy of insurance_ CONSULTANT shall pay, in a�roin and timely manner the premiums on the insurance hereinabove required. 11. INDEPENDENT CONTRACTOR CONSULTANT is, and shall be, acting at all times iii the performance of this Agreement as an independent contractor herein and not as an employee of CITY. CONSULTANT shall secure at its own cost and expense, and be responsible for any and all payment of all taxes, _.. _ _._...... ......... _ _.. social security, state disability insurance compensation, unemployment compensation and other payroll deductions for CONSULTANT and its officers, agents and employees and all business licenses, if any, in connection with the PROJECT and/or the services to be performed hereunder. 12. TERMINATION OF AGREEMENT All work required hereunder shall be performed in a good and workmanlike manner. CITY may tenninate CONSULTANT's services hereunder at any time with or without cause, and whether or not the PROJECT is fully complete. Any tennination of this Agreement by CITY shall agree/ surfnet/professional Svcs mayor 5 of 11 10/12 be lnade in writing, notice of which shall be delivered to CONSULTANT as provided herein. In the event of termination, all finished and unfinished documents, exhibits, report, and evidence shall, at the option of CITY, become its property and shall be promptly delivered to it by CONSULTANT. 13. ASSIGNMENT AND DELEGATION This Agreement is a personal. service contract and the work hereunder shall not be assigned, delegated or subcontracted by CONSULTANT to any other person or entity without the —p1�or-express i#te seat of CITY. If an-nsszg delegation or sibis approved, all --- --------------- approved assignees, delegates and subconsultants must satisfy the insurance requirements as set forth in Sections 9 and 10 hereinabove. 14- COPYRIGHTS/PATENTS CITY shall own all rights to any patent or copyright on any work, item or rnatedal produced as a result of this Agreement. 15. CITY EMPLOYEES AND OFFICIALS CONSULTANT shall employ no CITY official nor any regular CITY employee in the work performed pursuant to this Agreement. No -officer or employee of CITY shall have any financial interest in this Agreement in violation of the applicable provisions of the California Government Code, 16. SECTION HEADINGS The titles, captions, section, paragraph and subject headings, and descriptive phrases at the beginning of the various sections in this Agreement are merely descriptive and are included solely for convenience of reference only and are not representative of matters included or excluded from such provisions, and do not interpret, define, limit or describe, or construe the intent of the parties or affect the construction or interpretation of any provision of this Agreement. 20. INTERPRETA.TION OF TITS AGREEMENT The language of all parts of this Agreement shall in all cases be construed as a whole, according to its fair meaning, and not strictly for or against any of the parties. If any provision of this Agreement is held by an arbitrator or court of competent jurisdiction to be unenforceable, void, illegal or invalid, such holding shall not invalidate or affect the remaining agree/ surf ct/professional sves mayor % of 11 10/12 covenants and provisions of this Agreement- No covenant or provision shall be deemed dependent upon any other unless so expressly provided here. As used in this Agreement, the masculine or neuter gender and singular or plural number shall be deemed to include the other whenever the context so indicates or requires. Notting contained herein shall be construed so as to require the comrnission of any act contrary to law, and wherever there is any conflict between any provision contained herein and any present or future statute, law, ordinance or regulation contrary to which - - _--- parti�3�lra�e- tro right to this -------------- Agreement which is hereby affected shall be curtailed and limited only to the extent necessary to bring it within the requirements of the law. 21. ATTORNEY'S FEES In the event suit is brought by either party to construe, interpret and/or enforce the terms and/or provisions of this Agreement or to secure the performance hereof, each party shall bear its own attorney's fees, such that the prevailing party shall not be entitled to recover its attorney's 25. SURVIVAL Terms and conditions of this Agreement, which by their sense and context survive the expiration or termination of this Agreement, shall so survive. 26. CONSULTANT shall submit to CITY an invoice for each monthly payment due Such invoice shall: A) Reference this Agreement; B) Describe the services performed; C) Show the total amount of the payment due; D) Include a certification by a principal member of CONSULTANT'S firm that the work has been performed in accordance with the provisions of this Agreement; and _._ E) For all payments include an estimate of the percentage of work completed. Upon submission of any such invoice, if CITY is satisfied that CONSULTANT is making _ satisfactory- progress ... toward__compl.etion_.of_..tasks in accordance with this Agreement, CITY shall approve the invoice, in which event payment shall be made within thirty (30) days of receipt of the invoice by CITY. Such approval shall not be unreasonably withheld. If CITY does not approve an invoice, CITY shall notify CONSULTANT in writing of the reasons for non -approval and the schedule of performance set forth in Exhibit "A" may at the option of CITY be suspended until the parties agree that past performance by CONSULTANT is in, or has been brought into compliance, or until this Agreement has expired or is terminated as provided herein. 1 Exhibit B 5. Any billings for extra work or additional services authorized in advance and in writing by CITY shall be invoiced separately to CITY. Such invoice shall contain all of the information required above, and in addition shall list the hours expended and hourly rate charged for such time. Such invoices shall be approved by CITY if the work performed is in accordance with the extra work or additional services requested, and if CITY is satisfied that the statement of hours worked and costs incurred is accurate. Such approval shall not be unreasonably withheld. HB -372- PAGE 9 er We understand that the Engineering Division desires a Construction Management firm to manage construction and provide quality control, inspection, wage compliance on an as -needed basis for various capital improvement projects such as traffic signals, pump stations, curb, gutter, sidewalks, pipelines, structures, road rehabilitation, reservoirs, and bridge retrofits. The City anticipates awarding up to four contracts for an initial three (3) year term. CivilSource is committed to providing the requested services to the City in compliance with the Caltrans' Local Programs Manual and the City Quality Assurance Program (QAP) for Federal and State funded projects and all applicable prevailing wage laws including the Davis -Bacon and Related Acts. CivilSource's approach to construction support services is centered on our established policy of consistent and effective employee oversight. Though Amy Amirani, Principal, is ultimately responsible for the overall management of our resources, she depends on senior level and extensively experienced project managers to oversee the assigned construction support services team and ensure quality control in construction management and inspection services. Ms. Amirani and the project manager will closely monitor projects to verify that our construction managers and inspectors are performing within the guidelines of our established management and inspection procedures, which are patterned after the Caltrans Construction Manual. CivilSource takes this a step further by formulating and implementing a proactive public relations program and a concerted effort to identify previously unforeseen potential claims. CRITICAL ISSUES Based on our experience with capital improvement development projects, we have identified the following critical issues that will be addressed and defined on a per project basis: SENSITIVITY TO CONSTRUCTION IMPACTS to nearby residential neighborhoods, surrounding schools, and businesses. The CivilSource team is highly experienced in working with the public, City staff, and stakeholders. We will take a proactive approach in working with the public to keep them constantly informed of planned construction activities, and we will maintain consistent communication to ensure their concerns are noted and are immediately addressed. TRAFFIC CONTROL If not addressed properly, traffic control can become an issue during construction. Traffic control should be set to create a safe working ground for the contractor as well as vehicular and pedestrian traffic. With broad knowledge and experience in preparing traffic control plans for several projects (including within Caltrans ROW), the CivilSource team can ensure that all traffic control devices are set based on the approved traffic control plan as well as the CA MUTCD - 2010 Edition requirements and the Work Area Traffic Control Handbook (WATCH Manual). CIVILSOURCE, INC. HB -31731- Item 10. - 38 UTILITY COORDINATION We will address the potential for conflicts with existing utilities. If required, our Inspectors can coordinate potholing for major utility conflicts. Locating utility facilities prior to the start of construction will allow for early coordination with utility agencies for relocation of their facilities. Initial information gathering will result in ease of construction and avoidance of impacting the schedule or budget with unnecessary change orders or stand-by time. SLUT DOWN OPERATIONS Service interruptions may occur during the course of construction. We will take a proactive and sensitive approach in notifying all affected residents and businesses of when service will be interrupted. In addition, we will work closely with the Contractor to minimize interruption of service. We will coordinate any by-pass pumping requirements and shut down operations to ensure a smooth operation. PIPELINE CONSTRUCTION The CivilSource inspection team has extensive experience in the design, installation and inspection of all types of pipe construction. We understand the importance of following the design grades in pipelines and minimum velocity requirements for cleaning of gravity sewer mains. All pipe installations and repairs will follow industry standards; there will be no sags or low points in gravity pipes. IDIISPOSAL SE E AND C0 P U `:- T 1 7 PAP `C1 Staging areas, disposal sites, construction access, available space for trenchless methods and other construction related constraints will be analyzed prior to start of construction. Construction traffic can also be a significant issue within these residential areas. We will work closely with the Contractor to recommend staging and a detailed construction route to minimize congestion and noise in the community. MOBILITY Each member of our construction support services team has access to a mobile office that includes a laptop, Internet/email access, mobile phone, mobile project files, digital cameras and other required materials. Our team members are always available via mobile phone and email for easy access and continuous communication. SCHEDULE & CLAIMS ANALYSIS We can provide claims analysis including but not limited to the analysis of project schedules to detect and identify project float, critical path delays, concurrent and non -concurrent delays, compensable and non-compensable delays, forensic as -built schedule analysis and reconstruction, etc. We can also offer the City assistance in the preparation of specification provisions to minimize, mitigate and prevent to the extent practical any project related claims, and analyze claims upon receipt to determine the extent of the City's culpability and thus, liability. STATE/FEDERAL FUNDING For state and federally funded projects the City must comply with reporting requirements. Our team is experienced in complying with state and federal grant reporting and filing requirements, such as Community Development Block Grant, Section 3, Caltrans, EPA, State Water Resources Control Board, Energy Grants, etc. We pay close attention to these requirements so that project funding remains unaffected and so that file audits are conducted quickly and efficiently. Our team is also experienced in ensuring that project documents show compliance with DBE reporting requirements, that DBE goals are met, and/or good faith efforts documented. Item 10. - 39 1NC. HB -374- PACE 11 WAGE COMPLIANCE MONITORING CivilSource stays up-to-date with changes in construction -related job classifications and associated wage rates. We offer clients expertise in working with contractors to resolve wage issues through problem solving consultations, show -cause meetings, or if necessary, the labor relations hearing process. Our ultimate goal is to resolve wage issues at the lowest level possible before it evolves into a significant project dispute; thereby protecting the City from any liability of wage restitution. SCHEDULE CONTROL Our construction support services team gives considerable attention to monitoring project schedules. We will coordinate, review and make recommendations for changes to the Critical Path Method (CPM) schedule generated by the contractor. As changes or discrepancies occur between the CPM schedule and as -built conditions, the project schedule and cost estimates will be updated. We will review the contractor's schedule on a weekly basis and will maintain an as-built/working schedule to reflect as -built conditions to date. COST CONTROL AND REPORTING Cost control is a key element to successfully delivering a project and must be thoroughly integrated with other control methods (scope change control, schedule control and quality control) to ensure that the City receives full value for all funds earmarked for the project. Key elements of our cost control program include: establishment and maintenance of a project budget; value engineering and constructability reviews of project plans and specifications; sufficient vetting of owner requested or contractor generated changes; thorough review and evaluation of change order cost proposals; close monitoring of force account work; and regular maintenance of a change order log. BEST MANAGEMENT PRACTICES Our construction support services team is well versed in National Pollutant Discharge Elimination System (NPDES) requirements and experienced in implementing best management practices. We will monitor the implementation, maintenance and any required modification of the approved Stormwater Pollution Prevention Plan (SWPPP) and Water Quality Management Plan (WQMP). In the event that the project specifications for developing the SWPPP or WQMP are inadequate and cannot support the project needs or meet current NPDES requirements, we will work closely with the design consultants to incorporate the required provisions into the project specifications to meet these goals. In addition, our team includes Qualified SWPPP Developers and Qualified SWPPP Practitioners. In addition, we will be thoroughly familiar with the City's administrative policies; this responsibility includes: checking plans and specifications against requirements with which problems have occurred on similar jobs; comparing elevations, grades and details shown on plans as existing, with those at the actual site, reporting errors, omissions and deficiencies to the City; keeping a marked -up set of plans and specifications convenient for ready reference, and anticipating the contractor's operations by reviewing the plans and specifications for each operation. H13 -375- Item 10. - 40 • Before the start of construction, we will discuss with the contractor the definable features of work to ensure that documentation is complete, materials are on hand and that the workers understand the scope of work. • At the onset of work, we will perform an initial inspection to determine whether or not the contractor thoroughly understands and is capable of accomplishing the work as specified. Safety is also checked for proper implementation. The construction inspector will perform follow-up inspections on a daily basis. • We will ensure that the contractor has an established safety program and that regular safety meetings are conducted. We will also ensure that the contractor meets all OSHA safety requirements. • We will conduct a monthly review of the as -built drawings and ensure that all items are considered in the changes of the record drawings, including: size, type and location of existing and new utility lines; layout and schematic drawings of electrical circuits and piping; verification of alignment and cross sections; and changes in location of equipment. • Finally, we will review and approve the as -built drawings to complete the project turnover and begin the warranty period. Our first line of customer service comes from our inspectors. Our inspectors know the value of maintaining great relations with residents, business owners and anyone affected by a project, and they have proven to be a valuable resource in appeasing public concerns and resolving field issues. An effective and informative public outreach program that informs all affected residents, property owners and motorists about construction impacts and possible traffic delays is critical to the success of any project. If the City desires, CivilSource, as a part of its construction support services, can establish a 24-hour toll -free project hotline, linked directly to Amy Amirani who oversees all public outreach operations. We can also create a recorded message that includes project information and updates and give callers the option to leave messages. All messages will be responded to within 24 hours. We can also post project information and updates on the CivilSource website, which can be accessed via a link on the City's website. Updates and photos are posted on a weekly basis. Yorba Linda Boulevard Rehabilitation I City of Yorba Linda, Yorba Linda, CA February 2011 — April 2011. 2 mile primary arterial highway rehabilitation project. Improvements included asphalt overlay and reconstruction, curbs and gutters, cross gutters and spandrels, sidewalks, driveways and driveway approaches, wheel chair ramp designs meeting ADA requirements, striping, traffic signal loops, and traffic control. Sawtelle Boulevard Rehabilitation I City of Culver City, Culver City, CA A $1 million street rehabilitation and oversight of construction inspection services. The improvements included: total pavement reconstruction; the redesign of existing cross -sections, as needed, to improve drainage and drivability; and repair of damaged sections of the asphalt. La Bonita Park Water Facilities I City of La Habra, La Habra, CA Project Director. August 2011— July 2012. Ms. Amirani was responsible for the oversight of preparation of plans, specifications, and construction cost estimate for the La Bonita Park Water Facilities, including 250,000 gallon DYK below ground prestressed concrete tank, 14 MGD booster pump station, 850 GPM ground water well, 750kw generator, and sodium hypochlorite system. Site and grading plans were prepared for the facility. Galster Way Improvement Project I City of West Covina, West Covina, CA This project consists of pavement reconstruction on Galster Way from Westridge Road to approximately 50 feet west of the entrance of Galster Park. The project improvements include: the removal of deteriorated sections of pavement; the reconstruction of pavement; and the reconstruction of curb, gutter, cross -gutter, sidewalk, curb ramps and driveways. Corak Street Storm Drain Improvements I City of Baldwin Park, Baldwin Park, CA May 2013 — October 2015. Installation of 2,500 feet of new storm drain pipes on Ramona Boulevard between Earl Avenue and Francisquito Avenue, and along a drainage easement between Corak Street and Francisquito Avenue. The storm drains consists of reinforced concrete pipe ranging in size from 24-inches to 42-inches in diameter. The project also includes improvements to manholes, catch basins, transition structures, connector pipes, and junction structures. The project was put on hold for a period of time. La Bonita Park Water Facilities I City of La Habra, La Habra, CA Project Manager. August 2011 — July 2012. Design -build of a water facility that also included the demolition and reconstruction of a roller hockey rink, restroom/storage facility and parking areas. Professional services included the design and construction management of a new pre -stressed concrete reservoir, a new 11.5MGD pump station including new electrical and control facilities, intake and discharge piping, flow control facility, discharge metering vault, 2 miles of 24-inch cement mortar lined and coated pipe, and associated site improvements; construction oversight of traffic control, excavation and shoring, dewatering, relocation of existing sewer main, street improvements and pavement repairs, protection of all existing utilities and facilities; and facility start up and testing. The project also consisted of designing the equipping of the newly drilled well including a vertical turbine pump with 850-gpm capacity and 150-hp electrical motor, 2,500 ft of 8-inch PVC C900 discharge line and connection to La Bonita Pump stations, pump to waste line and connection to existing on -site storm drain system, and inclusive of all necessary instrumentation and controls required to operate the well. Well operation is also hydraulically controlled utilizing the CLA VAL well control and flow control with check feature valves. Total project Cost was $11M. 2013 DBIA Design -Build Distinction Award. Corak Street Storm Drain Improvements I City of Baldwin Park, Baldwin Park, CA May 2013 — October 2015. Installation of 2,500 feet of new storm drain pipes on Ramona Boulevard between Earl Avenue and Francisquito Avenue, and along a drainage easement between Corak Street and Francisquito Avenue. The storm drains consist of reinforced concrete pipe ranging in size from 24-inches to 42-inches in diameter. Paul Revere Park I City of Anaheim, Anaheim, CA Preparation of preliminary grading and drainage plans for the project based on the Landscape Architect and City approved conceptual site plans. The plans include pad elevations, site hardscape, contours, drainage inlets paving sections, fencing, and earthwork quantities. The plans also introduce recommendations on required BMP's to comply with water quality regulations and address any impacts to improvements within the City's right of way. CivilSource also prepared a preliminary cost estimate of the site improvements, including earthwork, hardscape and drainage improvements. His experience includes contract administration, CPM scheduling, claim dispute resolution, partnering, construction inspection, and material sampling and testing. Mr. Safa has successfully worked on construction projects in several states in the U.S. and in several countries overseas, under a variety of conditions. His experience in maintenance engineering has provided him with awareness to maintenance and operation issues, which benefit project owners. Mr. Safa also has design experience with consulting engineering firms. This experience enables him to effectively communicate with the design engineers to the benefit of his projects. Project Experience Construction Management, City of Garden Grove, Water Department, 2014-2016 As a construction manager and resident engineer oversaw the construction work in the upgrade of a water pump station consisting of three engine -driven booster pumps and an engine -driven well production pump. The work consisted of replacing engines and pumps with new equipment, as well as replacement of discharge piping, building modifications, and instrumentation. What made this project significant is the fact that all work was done with minimum disruption of service. The facility pumped water into the City's distribution system at all times, except short shut -down periods lasting only a few hours each time. Construction Management, City of Anaheim Water Utilities Department, 2012-2014 As a construction manager supported City staff in managing the construction of a 4 million gallon prestressed concrete reservoir, using DYK technology. The project also included the construction of two separate pumping stations for two different zones located in the same site. The value of the project was about $9 million. There was an existing deteriorated underground reservoir in place, coupled with existing pumps. The project was implemented in such a manner that production and distribution of water was maintained at all times, with minimum disrutption of service. This project was visible and the Resident Engineer worked with City staff to keep the neighbors well informed of the project progress, and solicited input from the public regarding the project. Construction Administration Consultant, Los Angeles Community College District, Los Angeles, CA, 2006 - 2011 As an independent consultant and engineer of record (EOR) provided construction administration services on a $30 million utility infrastructure improvement project on East Los Angeles College, provided oversight during the construction phase of the project. Additionally, provided utility coordination and support services on other projects on the college with an estimated construction cost of about $400 million. The FOR continued to provide support services well after the project completion, working with California reviewers, until all project documentation was completed to the satisfaction of the state reviewers, and until the project was fully certified by the state. The project had many challenges, but the end result is quite satisfactory to the project owner. Lake Forest Drive Street Resurfacing, 1-5 to Rockfield Boulevard, City of Lake Forest, CA Resident Engineer/Construction Manager for plans and specifications as well as contract administration and inspection during construction of the repavement of this extremely busy major arterial street located at the 1-5 freeway on- and off -ramps. HB -385- Item 10. - 50 Concrete Sewer Rehabilitation Program, South Gate, CA As Project Manager for engineering services, provided engineering services for a three-year prioritized program for rehabilitation of the City's sewer system. This phase of the sewer rehabilitation was funded by a South Gate Water Authority $30 million bond issue and will encompass the repair, replacement, and rehabilitation of approximately 320,000 linear feet of concrete sewer pipe. Deer Creek Wastewater Treatment Plant, El Dorado Irrigation District, El Dorado County, CA Resident Engineer for on -site professional construction management services and inspection for the expansion of plant capacity from 2.5 MGD to 3.6 MGD. The project included construction and renovation of sewage treatment plant facilities including a grit washer, secondary clarifier, gravity sludge thickeners, sludge storage facility improvement, sludge dewatering belt press installation, installation of lime sludge stabilization equipment, metal building, 1-million-gallon steel storage tank, related pumps and equipment, instrumentation and controls, pavement and other site work, and electrical power installations. The total project cost was approximately $5 million, including nearly $800,000 of owner -furnished equipment. City of Pomona Construction Management, Inspection Services, and Geotechnical Testing for Various Sewer Main Construction Projects Project Manager for on -site construction management and inspection services for approximately 10,000 LF of 12- and 15-inch sewer pipe with an approximate project cost of $2 million, in busy City streets in commercial and residential areas. Work included installation of tunnels under two railroad crossings. City of Pomona Construction Management, Inspection Services, and Geotechnical Testing for Various Water Main Construction Projects, CA Project Manager for Water Main Projects on Kenoak and Presidents South. Provided construction management and inspector services for several water pipeline projects with an approximate project cost of $1.5 million, in busy City streets in both commerce and residential areas. Work included resident contact and community outreach. Coachella Valley Water District (CVWD] Sewer and Water Inspection, Coachella, CA Project Manager for inspection services on a 1 MG steel water reservoir, water distribution pipelines, and sanitary sewer lines. These projects were constructed by developers with varying costs depending on the size of the tracts. Well No, 36 Pumping Equipment and Transmission Main Manifold, City of Pomona, CA Project Manager for design engineering and construction management services for a water transmission main that connects two existing wells (Well Nos. 21 and 24) and a new well (Well No. 36) to the City's anion exchange plant (AEP). The pipeline is sized to accommodate a future fourth well (Well No. 38). The City previously constructed Well No. 36 three years ago. Psomas is designing the pump and wellhead equipment for this well. Approximate project cost was $2 million. City of Lake Forest, El Toro Road Widening Project As a Project Manager during the construction phase of this important project, provided support to the City and oversaw the work of a resident engineer and construction inspector on the project. El Toro Road is a major arterial road with a heavy traffic load. Work consisted of widening the street, adding new sidewalks, relocating existing utilities and installing new utilities. Work also included landscaping and irrigation. This work was done in such a way that the public was allowed to use the street while under construction. Having good traffic control plans in place was very important to the implementation of this project. City of Laguna Beach Street Resurfacing and Sidewalk Improvements As Resident Engineer/Construction Manager provided services on road work projects on Glenneyre Street, and El Toro Road. The project was partially funded by Caltrans Local Projects funding, and required compliance with Caltrans procedures. The work was done in such a way that there was no complaints from the residents in that affluent community. Lake Forest Drive Street Resurfacing, Trabuco Road to Fernleaf Drive, City of Lake Forest, CA Resident Engineer/Construction Manager for the repavement of this extremely busy major arterial street traversing the City and linking Lake Forest to other communities. Work consisted of grinding old pavement, repavement, installation of traffic loops, striping to Caltrans standards, and installation of reflective plastic markers. The existing Sweany Reservoir outlet pipeline had a history of leaks and required replacement. The project installed a new 8-inch ductile iron pipe discharge line in rough terrains in the Emerald Canyon area, new fire hydrants, a connection to the existing reservoir at the depth of 25 feet below surface, sacrificial anode system, pipe anchors due to extreme slopes, and a connection to an existing PRV station. City and County of San Bernardino Senior Field Inspector and coordinator for a 3-year ADA Assessment Program for the County of San Bernardino County that included ten major Cities. The assessment included county courthouses, jails and juvenile facilities along with bus stop and parking lot accessibility reports. The reports were completed in conjunction with ADA Consultants, BOA located in San Pedro. Extensive field time was required to measure and map out all areas and note the discrepancies that were included in the final itemized cost reports. Bluff Restoration Project I City of Long Beach Senior Construction Manager for the stabilization of a washed out slope adjacent to homes and access road. The project involved the drilling and placement of 6 each -2' dia x 60' steel and concrete caissons. Pressure treated lagging was installed and the upper slope was re -compacted. Firestone Boulevard/Atlantic Avenue Street Improvements I City of South Gate, South Gate, CA Mr. Geyer provided construction management services for this project, which was the largest public works project in the City of South Gate to date at a cost of $10 million. The project limits were on two arterial streets: Atlantic Avenue from Firestone Blvd to Patata Street (approximately 1,300 linear feet) and Firestone Boulevard, from Dorothy Avenue to Atlantic Ave. (approximately 2,200 linear feet). The scope included widening the intersection, providing a concrete approach to the intersection, grind and ARHM overlay, 50,000 square feet of sidewalk, 1,850 linear feet of curb and gutter, four traffic signal modifications, 2,100 linear feet of DIP waterline, 2,840 linear feet of recycled waterline, 1,850 linear feet of 21-inch VCP sewer, various storm drain improvements, 4,656 linear feet of median island curb with decorative landscaping and a city monument sign. 1-5 Pre -Construction Mitigation Phase II 1 City of Santa Fe Springs, Santa Fe Springs, CA Mr. Geyer provided construction management services to the City of Santa Fe Springs for this $2.5 million, high -profile, essential, four -major arterial resurfacing/rehabilitation project. The scope of work included rehabilitating the roadways using primarily grind and overlay with 940 tons of conventional AC pavement. Construction also included 1.2 million square feet of cold milling with 15,050 tons of ARHM overlay, adjusting 69 manholes and 68 water valves, and 167 traffic signal loop detectors. There were 116 new or modified ADA compliant curb ramps installed with 3,550 linear feet of curb and gutter and 1029 square feet of sidewalks. Some of the work involved Caltrans right of way, oversight and coordination. Richfield Avenue Widening/RCB Improvements I City of Placentia, Placentia, CA Mr. Geyer provided construction management services for the construction of this project, which cost approximately $375,000. It extended the existing RCB (reinforced concrete box -double cell) at the Atwood Channel under Richfield Avenue, which was also widened. Telegraph Road Landscaped Medians I Cities of Downey and Pico Rivera, Downey & Pico Rivera, CA Mr. Geyer provided the construction management services for this project, which was funded by the Federal Highway Safety Improvement Program (HSIP) and Proposition C. The construction cost was $2.1 million and it included median, landscape, irrigation, roadway, parkway, traffic signal and peripheral improvements. The work was conducted on the east -west corridor of Telegraph Road, a residential and commercial major thoroughfare having approximately 26,000 vehicles per day and shared with the City of Downey. The work consisted of pavement and concrete removal under the medians, construction of 4,799 square feet of a concrete doweled -on median curb, construction of 430 square feet of four -inch sidewalk, curb and gutter construction, installation of concrete spandrels, 17 curb ramps and 21 truncated dome mats. In addition, the work entailed traffic signal modifications at Telegraph Road and Lakewood/Rosemead Boulevard, Telegraph Road and Serapis Avenue and Telegraph Road and Passons Boulevard. An alternate bid added enhancement and beautification items east from the end of the curb at Lakewood Boulevard and Telegraph Road to Passons Boulevard. Lincoln Boulevard Pavement Improvements I City of Santa Monica, Santa Monica, CA Mr. Geyer provided construction management services for this high -profile federally funded project on Lincoln Blvd, from the 1-10 to the south Santa Monica City limits. The estimated cost was $2.8 million funded by the Federal Transportation Grant. This portion of Lincoln Boulevard is an undivided arterial roadway that provides access through commercial portions of Santa Monica, intersecting other major arterials such as Pico Boulevard and Ocean Park Boulevard, while also serving as a primary travel route through the City, carrying traffic to multiple points including Marina Del Rey, Pacific Palisades and Los Angeles International Airport. The work entailed cold mill and ARHM overlay with full -depth reconstruction in some areas. Parkway improvements were also included as part of the project and encompassed curb, gutter, sidewalk, driveway and curb ramp replacement. The work included very extensive coordination with businesses and stakeholders. Construction took place at night and required careful coordination and cleanup. Harbor Boulevard/Adams Avenue Intersection Improvements I City of Costa Mesa, Costa Mesa, CA Mr. Geyer provided construction management services for the construction of this project, which was approximately $2.8 million and funded through Measure M2, the State -Local Partnership Program and Traffic Impact Fee Funds. Harbor Boulevard and Adams Avenue are heavily traveled arterials providing access to multiple businesses, shopping centers and Orange Coast College. This project provided a third dedicated left -turn lane eastbound on Adams Avenue resulting in major widening along the southerly side of Adams Avenue near the intersection approach. Additionally, it added a second dedicated right - turn lane southbound on Harbor Boulevard. Finally, this project also included bus stop relocations, decorative crosswalks at the Harbor/Adams intersection, traffic signal modifications, decorative crosswalks and sidewalks, median alterations, landscaping, and a general grind and overlay of the roadway. The work involved widening all four corners of the intersection, requiring extensive traffic signal modifications. Street and sidewalk rehabilitation. Inspect water infrastructure relocated to facilitate curb ramp changes or sewer installation. Utility protected in place like a 40 foot portion of GWP 16" water main was going to be supported as the sewer installation process past old cast iron, the installation crossed over at about 15 degrees with a 40 foot section exposed for the sewer installation. Before project began a by-pass pipeline was installed, flushed, and disinfected then after bacT samples passed work on main line began. I directed GWP crews to isolate and depressurized the water main. As contractor exposed water main every eight foot an eight inch H-beam was placed across ditch and wooden 4x4 were used between main and beam with 2x4 placed at each end of 44 for softeners, then all were bound together with straps and come -a -longs to create one unit. The trench was back filled with two sack slurry. Restoration of water main and services were completed. Water System Supervisor II - Mid -management position. Assume responsibility for construction, repair, maintenance and operation of water systems, pumping facilities. Supervised: Pump Maintenance Mechanics, Water Distribution Operators, Water Treatment Operators, SCADA Technician and Water System Apprentices. Oversee the operation and maintenance of a Diatomaceous Earth Filter Treatment Plant. Work with engineering on new Chlorination processes at Glorietta Park. Project Experience Lancaster 24-inch and 12-inch Recycled Water Pipeline Project I US Army Corps of Engineers, LA District, Lancaster, CA Design Engineer. Mr. Pham conducts field investigations, meetings and prepares final design materials for the design of approximately 4.5 miles of 24-inch recycled water main, 0.75 miles of 12-inch recycled water main and related facilities to serve the northern section of the City of Lancaster located in north Los Angeles County. In addition, Mr. Pham performs the work and services including documentation of coordination activities required to successfully produce all work in accordance with the scope of work. The design consists of contract drawings, specifications using Specs Intact, cost estimate [MCACES), and engineering considerations and instructions for field personnel. The design is prepared with sound engineering judgment, engineering design criteria (Los Angeles County Waterworks District No. 40), and followed procedures outlined in scope of work. Zone II — 24" Domestic Water Transmission Main ( Santa Margarita Water District, Mission Viejo, CA Mr. Pham was responsible for preparing the construction plans and specifications for over one mile of 24-inch CML&C steel water transmission main pipeline in Antonio Parkway, south of Windmill Avenue run south to O'Neill Drive which serving for Ladera development phase 1 and future phase located in City of Mission Viejo. He coordinated with engineers of Santa Margarita Water District to ensure the plan conformed to the Santa Margarita Water Standard. Due to the constraint schedule, Mr.Pham worked closely with the crew survey to ensure the plan was constructed on schedule. Shop drawings were also provided during the construction phase of this project. 60" Storm Drain Improvements on Lampson ( City of Garden Grove, CA Mr. Pham prepared the construction plans for approximately 1000 linear feet of 60" RCP Storm drain including final hydrology calculations, hydraulic analysis and calculation for sizing storm drain system. Due to unique underground existing utilities, he determined the alignment of the storm drain system and all appurtenances such as catch basins, laterals, and connector pipes as required. This task was involved existing utilities relocation and avoided any existing utilities has franchised ownership. Due to new storm drain system, street areas were regarded with removed cross gutter. At the same time, approximately 1000 linear feet of existing 10" sewer has been replaced in kind and in place by 15" due to deficiency report. Also according to the conflict of new alignment of proposed 60" storm drain, a portion of the existing water has been realigned. The plans of works included development of water and wastewater planning criteria using Santa Margarita Water District Standard for both residential and commercial land uses , hydraulic analyses and computer modeling for both water and sewer system. 16" and 24" Waterline Relocation I Long Beach Water Department, CA Mr. Pham provided design engineering services for realignment of 16" and 42" diameter water transmission main. In restricted commercial areas, the distribution pipelines were buried to five feet under the taxi run of Long Beach Airport. The pipelines were selected to handle both water supply and traffic load needs. Design included approximately 2,000 linear feet of 16" and 42" ductile iron. Downtown Anaheim Sewer Study I Community Redevelopment Agency, Anaheim, CA Mr. Pham performed an analysis of sewer flows tributary to and flowing from a proposed expansion to the downtown city center. The analysis performed determined the performance of eight miles of existing sewerage facilities downstream of the project to the existing trunk sewer for two mainline sewer systems, in both the existing and ultimate conditions. The purpose of the analysis was to determine if the existing sewerage facilities had or will have a pipe segment without adequate capacity for the anticipated sewer flows, recommended solutions to the city. Mr. Pham is responsible for providing engineering services to the City of Torrance for the design of three treatment wetlands throughout the City for storm water retention, treatment, and infiltration. Once constructed, these treatment wetlands will help the City comply with the Santa Monica Bay Beaches Bacteria Total Maximum Daily Load (TMDL) requirements.
