The 2005/2006 Form 700 is scheduled for approval by the Commission at the
December meeting and should be available on our website soon after January 1, 2006.
Due to ongoing budget constraints, we are not providing a hard copy of the form to cities
this year, unless filing officers. specifically request that we do so. Instead, we will send
you an e-mail notification informing you when the form is available on our website.
Please indicate your current e-mail address on the attached notice to ensure
that you will receive the e-mail notification. You may also wish to consult with your
agency's IT staff to take the proper steps so that our e-mail notification will not be
blocked. The e-mail will contain a memo updating you with important information you
need to know for the upcoming filing season. If you would like to request a hard copy of
the Form 700 and memo to be sent to you, please let me know.
Annual Notice of Filers
Enclosed with this mailing is the annual notice of filers whose statements should
be forwarded to the Commission. Please review the enclosed notice for accuracy and
indicate any changes before you return the notice to our office. Keep the following
points in mind when reviewing the list of filers:
1. The list should only include individuals in the following positions:
• Mayor • City Attorney
• City Council Members • City Manager
• Planning Commissioners • City Treasurer
• Chief Administrative Officer
2. Add the names and positions of any current filers who do not appear on the list.
Indicate the date (month, day and year) he or she assumed office.
3. Include the names and positions of any newly -elected (non -incumbent) officials.
Indicate the date (month, day and year) he or she will assume office.
4. If a name appears on the list and the person has left office, indicate the leaving
office date (month, day and year).
5. Please forward any assuming and leaving office statements for the positions listed
above to the FPPC as soon as possible, or let us know if an individual has not
filed so we may contact them. Remember that officials who are re-elected or re-
appointed are not required to file assuming and leaving office statements. These
officials continue filing annual statements. However, officials who are elected for
the first time must file assuming office statements within 30 days of assuming
office. The candidate statement filed in connection with their declaration of
candidacy does not suffice for the assuming office statement.
Filer Survey
We are compiling data regarding the number of Form 700 filers for each agency.
Please indicate the total number of filers for which you retain original statements under
your city's conflict -of -interest code on the cover page of the Article II List. Do not
include in your total statements which are forwarded to the FPPC.
