You will receive an e-mail in January with all the necessary information related to filing the Form 700. For a hard copy of the form, please e-mail or call me. Annual Notice of Filers Attached to this e-mail is the annual notice of filers whose statements should be forwarded to the Commission. Please review the notice for accuracy and keep the following points in mind: 1. The list should only include individuals in the following positions: • Mayor • City Attorney • City Council Members • City Manager • Planning Commissioners • City Treasurer • Chief Administrative Officer 2. Add the name and position of any current filer who does not appear on the list. Indicate the date (month, day and year) he or she assumed office. Make sure you include the names and assuming office dates for filers who were recently elected. If a name appears on the list and the person has left office, indicate the leaving office date (month, day and year), including those who have left office due to a recent election. 4. Remember that your city officials who are re-elected, re -appointed, or who assume another position within 45 days for a position listed above are not required to file assuming or leaving office statements. For example, a planning commissioner elected to city council simply continues to file annual statements. Be sure to note the title change on the Article II notice. Please forward any original assuming and leaving office statements to my attention. If you do not have a statement, let me know immediately so I can request it, and forward any correspondence you have sent to the official. 6. This notice is required to be returned even if there are no changes.
