SUBJECT: Adopt Fee Resolution No. 2010-33 for Revision of the City's FireMed
Fee Schedule for Emergency Medical Services
Statement of Issue: At the study session of March 1, 2010, the City Administrator and
Director of Finance presented proposed budget reductions and revenue enhancements
required to balance the FY 2009/10 general fund budget. Included in the revenue
enhancements was a proposal for the Fire Department to increase Basic Life Support and
Advanced Life Support fees and implement a non-resident fee for these services. This
request is the formal action for the Fire Department to implement these budget balancing
measures.
Funding Source: Not applicable.
Recommended Action: Motion to: Adopt Fee Resolution 2010 - 33, "A Resolution of the
City Council of the City of Huntington Beach Setting Advanced Life Support, Basic Life
Support, Emergency Ambulance Transportation, Medications and Specialized Supply Fees
to be Charged by the Fire Department of the City of Huntington Beach," amending
Resolutions No. 2009-80, "A Resolution of the City Council of the City of Huntington Beach
Setting Advanced Life Support, Basic Life Support, Emergency Ambulance Transportation,
Medications and Specialized Supply Fees to be Charged by the Fire Department of the City
of Huntington Beach."
Alternative Action(s):
1) Hold the Public Hearing and adopt only a portion of the fees.
2) Do not adopt any of the recommended fees.

Analysis: FireMed user fees were last reviewed by the City Council during a Public Hearing
held on December 21, 2009. At that time, a fee revision was requested to recover costs for
emergency medical supplies, equipment and personnel.
The user fees provide several benefits to the community by helping offset funding for:
• Firefighter Paramedic positions for improved emergency medical service
• Free CPR training to all FireMed members
• Ambulance Program costs
• Emergency Medical Service Quality Assurance Program
• Mechanic positions required for maintaining emergency vehicles
• Better customer care through the Emergency Medical Information Program
• Emergency medical service and transportation supplies and medications
• Improved operational readiness of City fire stations through paramedic equipment and
facility upgrades
City Ordinance section 8.68.110 allows revenues derived from the FireMed Program to be
used to improve and maintain the City's emergency medical services system. Revenues are
placed in the general fund to offset the costs associated with all medical related responses.
The proposed fee adjustments will allow the Fire Department to recover costs associated
with emergency medical services and activity in the Fire Department.
Revision to FireMed Emergency Medical Services Fee Schedule
Resolution 2010 - 33 (Attachment 1) includes the user fee schedule proposed by staff for
approval by the City Council. A Notice of Public Hearing (Attachment 2) was published. This
schedule represents fees for emergency transport, basic life support (BLS), advanced life
support (ALS), supplies and medications, additional specialized ALS services, emergency
medical assessment and membership rates, effective May 5, 2010.
Basic and Advanced Life Support Services
The proposed user fees for ALS and BLS service levels ensure adequate deployment of
paramedic resources by recovering costs from various insurance companies, government
agencies, non-member users, and uninsured FireMed members at a discounted rate.
BLS service represents the level of care provided by an Emergency Medical Technician
(EMT), who is an employee with certification provided by a certifying authority such as the
State Fire Marshal's Office and the Orange County Emergency Medical Services Agency
(OCEMS). In Orange County, the scope of practice includes assessment, basic airway
maneuvers, CPR, relief of airway obstruction, and other treatments as defined by OCEMS
Policy 315.00 covering scope of practice for EMTs.
All firefighters are certified as Emergency Medical Technicians. They must attend an
approved training program that includes a minimum of 110 hours of training and must
successfully pass the certification examination. Continued education requires an additional

24 hours of continuing education every two (2) years and completion of sixteen skill
competency exams.
ALS service is a higher level of emergency medical care and is provided by an Emergency
Medical Technician-Paramedic. Licensure requires attendance at an accredited paramedic
school and successful completion of the National Registry exam with proven skills
competency. Skills include advanced airway maneuvers, invasive treatments such as
establishing intravenous lines, chest decompression, administration of medications,
recognition of heart arrhythmias and defibrillation. Approximately 40% of firefighters are
certified as paramedics. Initial training requires 1,064 hours with an additional 48 hours of
continuing education every two (2) years. In Orange County, Paramedics must also attend
an annual airway training class and complete a skills competency exam.
The proposed increases in BLS and ALS fees are based on the following impacts:
• Loss of revenue as a result of recent changes in Medicare reimbursements; Medicare
patients represent 48% of patients treated by the Fire Department
• Increased salary and benefit costs
• Higher renewal and processing fees for EMT and Paramedic licensing
• Greater costs for background processing to comply with state and federal mandates
• Additional training costs for EMTs to comply with the broader scope of skills required
to practice in Orange County
• Additional training costs for compliance with new emergency therapeutic interventions
provided by Paramedics
• New requirements to provide additional vaccinations for Fire Department employees
to comply with OSHA mandates
Other Orange County cities charge fees for BLS and ALS services; Table 1 compares the
Orange County cities surveyed.

The FireMed Fee Schedule for BLS and ALS rates has not increased in seven (7) years,
since 2003. However, the City Council has approved annual increases for emergency
ambulance charges, supplies and medications during this time. These increases followed
the adjustments approved by the Orange County Board of Supervisors for the OCEMS
Agency. Since 2004, rates for ALS services provided by the Orange County Fire Authority to
22 cities and all unincorporated areas in the County have increased by the same percentage
change as that for the maximum emergency BLS rates. Table 2 shows how a similar
adjustment would support the increase of the current FireMed Fee Schedule for the ALS rate
to $450. The projected revenue for this fee is $37,500 for the remainder of this fiscal year,
with a projection of $75,000 for FY 2010/11.

NON-RESIDENT TRANSPORTATION FEE
Approximately 33% of the patients treated in emergency medical situations by the Huntington
Beach Fire Department are non-residents. Over 16 million people visit Huntington Beach
each year, and this impacts the emergency medical service provided to the community. The
service demand by non-residents stresses the system and requires additional staffing to
support the increased call volume. Residents pay property taxes which partially fund the
emergency medical service provided by the Huntington Beach Fire Department. However,
non-residents do not pay property taxes and yet compel the department to maintain an
emergency medical services system that is available and quick to respond.
For example, during the summer months of 2009, an additional ambulance was placed into
service during peak demand, increasing the number of fire department ambulances in the
City from four (4) to five (5). An example of the impact occurred during the US Open of
Surfing on a Saturday afternoon. Despite having five (5) ambulances in service in the city
and two (2) additional ambulances covering the contest, all ambulances were committed to
emergency situations at the same time and a private ambulance was requested for a medical
emergency. Deploying an additional ambulance during the busy summer months was
successful in ensuring that the community receives quick emergency medical responses,
and will continue in the future.
There is an increased demand on the emergency medical service attributed to non-resident
patients. A non-resident fee will help finance the staffing that is required to allow the system
to continue to provide quick emergency medical response and treatment. An average
emergency medical incident, including treatment, transportation, reports and associated
paperwork, takes approximately 45 minutes of service by a paramedic engine company and

one hour by a fire department ambulance. The cost of the manpower to provide an
emergency medical response and transport is based on the approved Fire Department fully
burdened rate sheet developed by Public Resource Management Group and adopted, by
reference, in City Resolution 2009-31. Table 3 shows the calculation of the cost of this
manpower to support a Non-Resident Fee of$450.

A non-resident transportation fee would be charged only when a non-resident is treated at an
incident location and transported to a local hospital. The projected revenue for this fee is
$112,500 for the remainder of this fiscal year, with a projection of $225,000 for FY 2010/11.
A number of cities in California charge a non-resident fee, ranging from $96 by the City of
Coronado to $542 by the City of Vacaville. Table 4 shows the proposed non-resident
transportation fee and the fee charged by five other cities in Orange County. With approval
of this fee, Huntington Beach will not have the highest fee in the county.

Supplies and Medications
In the FireMed User Fee Schedule A for Advanced Life Support Supplies and Medications,
staff proposes a decrease in the fee charged for "Fabric Blanket", an existing item that was
reduced in cost due to recent bid results. All other supplies and medications fees remain the
same. Supplies and medications are priced at actual cost recovery plus administrative
processing.
In summary, the new rate schedule permits the City's emergency medical service programs
to maintain current levels of service while reducing economic impacts on the General Fund.
Staff recommends the adoption of the revised rate structure contained in the attached
resolution.

A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF HUNTINGTON BEACH SETTING ADVANCED LIFE SUPPORT,
BASIC LIFE SUPPORT, EMERGENCY AMBULANCE TRANSPORTATION,
MEDICATIONS AND SPECIALIZED SUPPLY FEES TO BE CHARGED BY
THE FIRE DEPARTMENT OF THE CITY OF HUNTINGTON BEACH
WHEREAS, Section 8.68.070 of the Huntington Beach Municipal Code provides that the
City Council shall by resolution establish fees for all recipients of Advanced Life Support, Basic
Life Support and Emergency Ambulance Transportation services provided by the City of
Huntington Beach Fire Department; and
FireMed user fees were last revised by the City Council pursuant to Resolution No. 2009-80;
and
In order to assist in defraying the cost of paramedic and ambulance services, the City
Council desires to revise fees to be paid by persons who utilize or benefit from having said
paramedic and ambulance services readily available; and
The fees hereinafter established do not exceed the estimated reasonable cost of providing
paramedic and emergency ambulance services in the City of Huntington Beach for which the fees
are charged,
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Huntington
Beach that the fees set forth herein below and on the attached Schedule A are hereby established as

Exemption And Reduction Of Charges
Determination for an exemption or reduction of fees shall be made by the Fire Chief. His
findings shall be based on the following criteria:
(a) FireMed Membership Fees—Low Income Household:
Persons who wish to participate in the FireMed Membership program but are unable
to pay the full membership fee because of hardship conditions may enroll at the
"Low Income Household" rate. Determination shall be based on "HUD Income
Guidelines — Very Low Income Category" currently on file at the City's office of
Economic Development.
(b) User Fees—Inability to Pay:
Persons receiving emergency medical services from the Huntington Beach Fire
Department and claim an inability to pay may apply to the Fire Chief for a reduction
of charges. The Fire Chief will evaluate the availability of insurance, government
assisted programs and total household income to determine the reduction of fees.
The Fire Chief may reduce fees to no less than the annual membership fee.
BE IT FURTHER RESOLVED that the effective date of the fees set forth in this resolution
shall be on May 0 % 2010 Nothing contained herein shall prohibit the combining of any
Fire Department ALS/BLS, ambulance transportation, medication, or supply fee for billing
purposes.
BE IT FURTHER RESOLVED that all resolutions in conflict herewith are hereby repealed.

REVISION TO THE CITY'S FIREMED
EMERGENCY MEDICAL SERVICES FEE SCHEDULE
NOTICE IS HEREBY GIVEN that a public hearing will be held by the City Council of the
City of Huntington Beach in the City Council Chambers of the Civic Center located at
2000 Main Street, Huntington Beach, California, at the hour of 6 p.m. or as soon
thereafter as possible on Monday, the 19t" of April 2010, for the purpose of considering
revisions to the City's FireMed user fee schedule. All proposed revisions are cost
recovery in nature.
It is proposed that a new rate schedule be adopted which will require higher fees to be
paid by persons, or their respective insurance companies, that use the City's
Emergency Medical Services system.
The types of fees under consideration for revision include emergency ambulance
transportation, specific medications, and related paramedic supply charges. The
Huntington Beach Fire Department is proposing a revised user fee schedule that
recovers the increased cost of these items or services.
A complete report detailing proposed revisions to the FireMed Emergency Medical
Services fee schedule is available for review by the public at City Hall, 2000 Main
Street, in the City Clerk's office between the hours of 8:00 a.m. and 5:00 p.m., Monday
through Friday. Interested persons may also contact the Fire Department at 2000 Main
Street, Huntington Beach, California 92648 to receive additional information.
All citizens are invited to attend the public hearing and provide the City Council with
written and oral comments or ask questions concerning proposed revisions to the City's
FireMed Emergency Medical Services fee schedule.

REVISION TO THE CITY'S FIREMED
EMERGENCY MEDICAL SERVICES FEE SCHEDULE
NOTICE IS HEREBY GIVEN that a public hearing will be held by the City Council of the
City of Huntington Beach in the City Council Chambers of the Civic Center located at
2000 Main Street, Huntington Beach, California, at the hour of 6 p.m. or as soon
thereafter as possible on Monday, the 19th of April 2010, for the purpose of considering
revisions to the City's FireMed user fee schedule. All proposed revisions are cost
recovery in nature.
It is proposed that a new rate schedule be adopted which will require higher fees to be
paid by persons, or their respective insurance companies, that use the City's
Emergency Medical Services system.
The types of fees under consideration for revision include emergency ambulance
transportation, specific medications, and related paramedic supply charges. The
Huntington Beach Fire Department is proposing a revised user fee schedule that
recovers the increased cost of these items or services.
A complete report detailing proposed revisions to the FireMed Emergency Medical
Services fee schedule is available for review by the public at City Hall, 2000, Main
Street, in the City Clerk's office between the hours of 8:00 a.m. and 5:00 p.m., Monday
through Friday. Interested persons may also contact the Fire Department at 2000 Main
Street, Huntington Beach, California 92648 to receive additional information.
All citizens are invited to attend the public hearing and provide the City Council with
written and oral comments or ask questions concerning proposed revisions to the City's
FireMed Emergency Medical Services fee schedule.
