SUBJECT: Adopt Resolution No. 2012-11 revising the City's FireMed Fee
Schedule for Emergency Medical Services
Statement of Issue: The Fire Department is requesting approval to adjust user fees to
improve and/or maintain the Fire Department's emergency medical services.
Financial Impact: An addition of $63,000 in general fund revenue is projected for FY
2011/12, with $94,500 anticipated in FY 2012/13. This revenue will offset increased costs
associated with providing emergency medical services to Huntington Beach.
Recommended Action: Motion to:
Adopt Resolution No. 2012-11, "A Resolution of the City Council of the City of Huntington
Beach Setting Advanced Life Support, Basic Life Support, Emergency Ambulance
Transportation, Medications and Specialized Supply Fees to be Charged by the Fire
Department of the City of Huntington Beach."
Alternative Action(s):
1) Hold the Public Hearing and adopt only a portion of the fees.
2) Do not adopt any of the recommended fees.
Analysis: FireMed user fees were last reviewed by the City Council during a Public Hearing
held on May 3, 2010. At that time, a fee revision was requested to recover costs for
emergency medical supplies, equipment and personnel.
The user fees provide several benefits to the community by helping offset expenditures for:
• Firefighter Paramedic positions for improved emergency medical service
• Free CPR training for all FireMed members
• Ambulance Program costs
• Emergency Medical Service Quality Assurance Program
• Better customer care through the Emergency Medical Information Program
• Emergency medical service and transportation supplies and medications

• Improved operational readiness of City fire stations through paramedic equipment and
facility upgrades
City Ordinance section 8.68.110 allows revenues derived from the FireMed Program to be
used to improve and maintain the City's emergency medical services system. The proposed
fee adjustments, with additional revenues placed in the general fund, will allow the Fire
Department to offset increased costs associated with providing emergency medical services
to the community by lessening impacts such as:
• Uncompensated supply and medication costs resulting from the restrictive 2002
Medicare fee schedule (which represents 49% of patients treated by the Fire
Department) and Medi-Cal (10% of patients)
• Nationwide shortages and increased cost of raw materials leading to higher costs for
emergency supplies and medications
• State licensing changes creating additional EMT processing fees
New Orange County Emergency Medical Services Agency (OCEMSA) policies
allowing more intensive patient treatments creating additional training costs
• Compliance with state-of-the-art medical procedures and electronic reporting
technologies producing additional instructional expenses
Revision to FireMed Emergency Medical Services Fee Schedule
The resolution shown as Attachment 1 includes the user fee schedule proposed by staff for
approval by the City Council. A Notice of Public Hearing (Attachment 2) was published. This
schedule represents fee adjustments for emergency transportation, medications and
specialized supplies. For the remainder of FY 2011/12, these changes will provide
approximately $63,000 in additional general fund revenue to the city, with $94,500
anticipated in FY 2012/13.
Adjustment to Emergency Transportation Rates
To recover increased operating costs, staff proposes to adjust existing emergency
transportation fees to coincide with the OCEMSA emergency basic life support ambulance
rates overall adjustment of 5.9% authorized by the County of Orange Board of Supervisors,
effective December 12, 2011 (Attachment 3). These rates, which have not been adjusted by
the Board in over three and a half years, establish the maximum rate schedule for private
ambulance ground transportation services. Current and proposed Huntington Beach
emergency transportation fees are compared with the approved Orange County ambulance
rates in Table 1 that follows.

OCEMSA determined the rate adjustments by applying the Los Angeles-Riverside-Orange
County "All Items" Consumer Price Index multiplied by 150% to offset costs associated with
salary, equipment, training, communications equipment, fuel, maintenance/repair, ambulance
replacement, etc.
Supplies and Medications
Schedule A for Medications and Specialized Supplies is updated with the current inventory
used to provide emergency medical services. Due to innovative patient treatment
procedures and state-of-the art technology, these lists reflect the associated medications and
supplies. Pricing fluctuates based on new technology, lower cost alternatives, increased cost
of raw materials, nationwide shortages and inventory control. All medications and
specialized supplies were reviewed for cost variability since the last fee resolution, and are
priced at the current actual cost or recovery plus administrative processing.
Supplies or medications represent new innovations such as a medical treatment procedure
that lowers a patient's body temperature to improve survivability, a device which provides
atomized medication for faster and more effective drug delivery, positive pressure ventilation
therapy tools for patients in respiratory distress and a nausea medication.
Staff proposes a 0.5% increase for existing medications listed in the FireMed Fee Schedule
and an overall 4% increase when the new medications are included. The current specialized
supplies fee schedule decreased by 7%, but with the addition of new supplies the adjustment
proposed by staff to offset the cost is a 1% overall increase.
In summary, the new rate schedule permits the City's emergency medical service programs
to maintain current levels of service while reducing economic impacts on the General Fund.
Staff recommends the adoption of the revised rate structure contained in the attached
resolution. A PowerPoint presentation is included as Attachment 4.

A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF HUNTINGTON BEACH SETTING ADVANCED LIFE SUPPORT,
BASIC LIFE SUPPORT, EMERGENCY AMBULANCE TRANSPORTATION,
MEDICATIONS AND SPECIALIZED SUPPLY FEES TO BE CHARGED BY
THE FIRE DEPARTMENT OF THE CITY OF HUNTINGTON BEACH
WHEREAS, Section 8.68.070 of the Huntington Beach Municipal Code provides that the
City Council shall by resolution establish fees for all recipients of Advanced Life Support, Basic
Life Support, Emergency Ambulance Transportation, Medications and Specialized Supply services
provided by the City of Huntington Beach Fire Department; and
FireMed user fees were last revised by the City Council pursuant to Resolution No. 2010-33;
and
In order to assist in defraying the cost of paramedic and ambulance services, the City
Council desires to revise fees to be paid by persons who utilize or benefit from having said
paramedic and ambulance services readily available; and
The fees hereinafter established do not exceed the estimated reasonable cost of providing
paramedic and emergency ambulance services in the City of Huntington Beach for which the fees
are charged,
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Huntington
Beach that the fees set forth herein below and on the attached Schedule A are hereby established as
follows:

Exemption And Reduction Of Charges
Determination for an exemption or reduction of fees shall be made by the Fire Chief. His
findings shall be based on the following criteria:
(a) FireMed Membership Fees—Low Income household:
Persons who wish to participate in the FireMed Membership program but are unable
to pay the full membership fee because of hardship conditions may enroll at the
"Low Income Household" rate. Determination shall be based on "HUD Income
Guidelines — Very Low Income Category" currently on file at the City's office of
Economic Development.
(b) User Fees—Inability to Pay:
Persons receiving emergency medical services from the Huntington Beach Fire
Department and claim an inability to pay may apply to the Fire Chief for a reduction
of charges. The Fire Chief will evaluate the availability of insurance, government
assisted programs and total household income to determine the reduction of fees.
The Fire Chief may reduce fees to no less than the annual membership fee.
BE IT FURTHER RESOLVED that the effective date of the fees set forth in this resolution
shall be on February 23, 2012. Nothing contained herein shall prohibit the combining of any Fire
Department ALSBLS, ambulance transportation, medication, or supply fee for billing purposes.
BE IT FURTHER RESOLVED that all resolutions in conflict herewith are hereby repealed.
