SUBJECT: Adopt Resolution No. 2012-61 revising the City's Emergency Medical
Services Fee Schedule
Statement of Issue: The Fire Department is requesting approval to adjust user fees to
improve and/or maintain the Department's emergency medical services.
Financial Impact: Not applicable.
Recommended Action: Motion to:
Adopt Resolution No. 2012-61 , "A Resolution of the City Council of the City of Huntington
Beach Setting Advanced Life Support, Basic Life Support, Emergency Ambulance
Transportation, Medications and Specialized Supply Fees to be Charged by the Fire
Department of the City of Huntington Beach.
Alternative Action(s):
1) Hold the Public Hearing and adopt only a portion of the fees.
2) Do not adopt any of the recommended fees.
Analysis: FireMed emergency medical service user fees were last reviewed and approved
by the City Council in March 2012. At that time, the change coincided with the fee schedule
of the Orange County Emergency Medical Services Agency (OCEMSA) to recover operating
costs for emergency ambulance service, which they had not increased since 2009.
The user fees help offset expenditures that provide the following benefits to the community:
• Improved emergency medical services
• CPR training for FireMed Program members
• Ambulance Program operating costs
• Medical Quality Assurance Program, which oversees firefighter paramedic
training and certification
• Emergency medical treatment and transportation supplies and medications
• State of the art paramedic and ambulance equipment

City Ordinance Section 8.68.110 allows revenues derived from the FireMed Program to be
used to improve and maintain the City's emergency medical services. The proposed fee
adjustment revenues will be placed in the General Fund, allowing the Fire Department to
recover the increased costs associated with providing exemplary emergency medical
service to the community. This reduces impacts, such as:
• Uncompensated supply and medication costs resulting from restrictive Medicare
and Medical fee schedules (representing 59% of patients treated by the Fire
Department)
• Higher costs for emergency supplies and medications created by nationwide
shortages and increased cost of raw materials
• Additional training costs to accommodate new OCEMSA policies which allow
more intensive patient treatments
• Compliance with state-of-the-art medical procedures and electronic reporting
technologies producing additional instructional and equipment expenses
REVISION TO EMERGENCY MEDICAL SERVICES FEE
The resolution, shown as Attachment 1, includes the user fee schedule proposed by staff
for approval by the City Council. A Notice of Public Hearing (Attachment 2) has been
published. This fee schedule represents adjustments for: 1) Emergency Transportation
and 2) Medications and Specialized Supplies. For Fiscal Year 2012/13, these adjustments
will provide approximately $50,000 of additional general fund revenue to offset the cost of
providing quality emergency medical services to the community.
1) Adjustments to Emergency Transportation Rates
To match rates allowed for private ambulance service in Orange County and to recover
operating costs, staff proposes to adjust existing emergency transportation fees to coincide
with the OCEMSA emergency basic life support ambulance rates overall adjustment of
2.7%. This was authorized by the County of Orange Board of Supervisors, effective July 1,
2012 (Attachment 3). These rates establish the maximum rate schedule for all private
ambulance ground transportation services in Orange County. Current and proposed
Huntington Beach emergency transportation fees are equal to the approved Orange County
ambulance rates, as shown in Table 1 that follows.

2) Medications and Specialized Supplies
Schedule A (included in the attached resolution) is updated with the current inventory used
to provide emergency medical services. It includes the items approved by OCEMSA which
are needed for innovative patient treatment procedures and state-of-the-art technology.
Pricing fluctuates based on new technology, lower cost alternatives, increased cost of raw
materials, nationwide shortages and inventory control. All medications and specialized
supplies were reviewed for cost variability and are priced at the current actual cost of
recovery plus administrative processing. Compared to the last fee resolution, City staff
proposes a 1% increase for medications and a 6% increase in fees for specialized supplies,
as listed in Schedule A.
In summary, the new rate schedule permits the City's emergency medical service programs
to maintain and enhance service while reducing economic impacts on the General Fund.
Staff recommends the adoption of the revised rate structure contained in the attached
resolution. A PowerPoint presentation is included as Attachment 4.

A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF HUNTINGTON BEACH SETTING ADVANCED LIFE SUPPORT,
BASIC LIFE SUPPORT, EMERGENCY AMBULANCE TRANSPORTATION,
MEDICATIONS AND SPECIALIZED SUPPLY FEES TO BE CHARGED BY
THE FIRE DEPARTMENT OF THE CITY OF HUNTINGTON BEACH
WHEREAS, Section 8.68.070 of the Huntington Beach Municipal Code provides
that the City Council shall by resolution establish fees for all recipients of Advanced Life
Support, Basic Life Support, Emergency Ambulance Transportation, Medications and
Specialized Supply services provided by the City of Huntington Beach Fire Department;
and
FireMed user fees were last revised by the City Council pursuant to Resolution
No. 2012-11; and
In order to assist in defraying the cost of paramedic and ambulance services, the
City Council desires to revise fees to be paid by persons who utilize or benefit from
having said paramedic and ambulance services readily available; and
The fees hereinafter established do not exceed the estimated reasonable cost of
providing paramedic and emergency ambulance services in the City of Huntington Beach
for which the fees are charged,
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Huntington Beach that the fees set forth herein below and on the attached Schedule A are
hereby established as follows:

Exemption And Reduction Of Char es
Determination for an exemption or reduction of fees shall be made by the Fire
Chief. His findings shall be based on the following criteria:
(a) FireMed Membership Fees—Low Income Household:
Persons who wish to participate in the FireMed Membership program but
are unable to pay the full membership fee because of hardship conditions
may enroll at the "Low Income Household" rate. Determination shall be
based on "HUD Income Guidelines — Very Low Income Category"
currently on file at the City's office of Economic Development.
(b) User Fees—Inability to Pay:
Persons receiving emergency medical services from the Huntington Beach
Fire Department and claim an inability to pay may apply to the Fire Chief
for a reduction of charges. The Fire Chief will evaluate the availability of
insurance, government assisted programs and total household income to
determine the reduction of fees. The Fire Chief may reduce fees to no less
than the annual membership fee.
BE IT FURTHER RESOLVED that the effective date of the fees set forth in this
resolution shall be on September 18, 2012. Nothing contained herein shall prohibit the
combining of any Fire Department ALSBLS, ambulance transportation, medication, or
supply fee for billing purposes.
BE IT FURTHER RESOLVED that all resolutions in conflict herewith are hereby
repealed.

This is to inform you that the Orange County Board of Supervisors approved the following revised
emergency basic life support ambulance rates at its July 24, 2012 meeting.

SCHEDULE
NOTICE IS HEREBY
GIVEN that a public
hearing will be held by
the City Council of the
City of Huntington Beach
in the City Council
Chambers of the Civic
Center located at 20GO
Main Street, Huntington
Beach,California,at the
hour of 6 p.m. or as
soon thereafter as
possible on Monday,the
17th of September,
2012,for the purpose of
considering a revision of
Fee Resolution 2012-11
regarding city-wide user
fees. All proposed revi-
sions are cost recovery
in nature.
It is proposed that the
previously adopted user
fee schedule be revised
for Fire Department user
fees. The types of fees
under consideration
include those associated
with Fire Department
inspections.
A complete report de-
tailing the proposed fee
schedule is available for
review by the public on
Thursday,September 13,
2012, at City Hall, 2000
Main Street, in the City
Clerk's Office between
the hours of 8:00 a.m.
and 5:00 p.m., Monday
through Friday. Inter-
ested persons may also
contact the Fire De
partment at 2000 Main
Street, Huntington
Beach, California 92648
to receive additional
information.
All citizens are invited
to attend the public
hearing and provide the
