Lewis #1182 2000 Main Street, Huntington Beach, CA 92648   Background On Saturday, September 25, 2021, at approximately 1605 hours, I was notified by Orange County Sheriff’s Department (OCSD) Homicide Sergeant A. Abdelmuti #5428 of an officer involved shooting (OIS) involving officers from the Huntington Beach Police Department (HBPD) and a deceased male subject who was later identified as, At approximately 1730 hours, I arrived at the incident command post located east of the HBPD sub-station, 285 Pacific Coast Highway in the City of Huntington A copy of the "true" original shall be obtained and released by Records Division Page 1 of 3 Printed by Inform RMS System Account on 10/22/2021 08:57 COPY DR #: 21-032545 Reporting Area: Homicide SUPPLEMENT Beach [County of Orange, State of California]. At approximately 2335 hours I arrived at UCI Medical Center located at UCI Medical Center, 101 The City Drive South in the City of Orange [County of Orange, State of California]. Upon my arrival, I contacted HBPD Detective J. Kearby #1010 in the ED (Emergency Department) Annex Room #20. Detective Kearby was watching the body of since approximately 1623 hours. For further details, see his report under HBPD DR 21-012353. Evidence Provided by Detective Kearby Prior to entering ED Annex Room #20, Detective Kearby provided me with a brown paper bag containing all the clothing found on person. Detective Kearby said the bag containing the clothing for was given to him when he arrived at 1623 hours to relieve HBPD Officers Colin Lewis #2079 and Dylan Morris #2126. Officers Lewis and Morris were with when he was transported to UCI from the scene in Huntington Beach and remained with until the arrival of Detective Kearby. ED (Emergency Department) Annex Room #20 Upon entering ED Annex Room #20, Detective Kearby and I were joined by Deputy Coroner , Forensic Scientist , Forensic Specialist and HBPD Detective J. Lewis #1182. body was located lying on the room’s only hospital bed and was covered by a white hospital sheet. When Deputy Coroner removed the sheet covering body, he was lying in a supine position. He had multiple medical pads and tubing attached to his chest, legs, arms and mouth. He also had a tourniquet attached to his upper left leg. Deputy Coroner further inspected body and noted the following: -          Eighteen (18) holes consistent with a possible gunshot wound -          Two (2) grazing injuries consistent with a possible gunshot wound After conducting the preliminary examination of body, Deputy Coroner sealed him into a Coroner body bag at 0154 hours (Sunday, September 26, 2021); seal number was then transported to the Orange County Coroner’s Office located at 1071 W. Santa Ana Blvd in the City of Santa Ana [County of Orange, State of California]. For further details, see Forensic Report number 21-52599 and Coroner case number 21-05698-LM. ED (Emergency Department) Annex Room #20 During the preliminary examination of body, Forensic Scientist took possession of the brown paper bag containing the clothing found on person at the time of the OIS. Forensic Scientist conducted an inventory of the bag’s contents which included: (1) white t-shirt, (1) blue t-shirt, (1) blue short, (1) gray boxers, and (1) pair of white socks. No personal property was found on body nor in his clothing. For further details on the booking of clothing refer to Forensic Report number 21-52599.
