Narrative:
On Saturday, September 25, 2021, at about 1512 hours, an Officer Involved Shooting [OIS] occurred
in the area of 285 Pacific Coast Highway in the City of Huntington Beach. The officers involved were
subsequently transported to the Huntington Beach Police Station located at 2000 Main Street in the
City of Huntington Beach. A short time later, Investigators with the Orange County Sheriff’s
Department responded to investigate the OIS.
I initially responded to the scene located at 285 Pacific Coast Highway for a briefing of the incident.
After the briefing, Forensic Specialist (FS) from the Orange County Crime Lab
(OCCL) and I responded to the Huntington Beach Police Department located at 2000 Main Street in
the City of Huntington Beach to process the officers who were involved in the OIS. The officers

involved were as follows; Detective R. Parker #1041, Detective C. Remington #1043, Officer D.
Velasco #2045, Officer C. Simpson #3013, and Officer N. Koehler #2263.
Upon our arrival and prior to the processing of the involved officers, HBPD Sgt. Burton provided me
with a USB Drive containing Huntington Beach Police radio traffic, dispatch phone calls, dispatch
logs and radio traffic.
Huntington Beach Detective Parker:
At about 1950 hours, FS photographed Detective Parker as he appeared at the time of the
shooting (dressed in police uniform) inside of the Hunting Beach Detective Conference Room. FS
also inventoried and collected the following property from Parker:
On Parker’s duty belt was a Glock, Model 17, 9MM handgun, serial number The handgun
was loaded with one round of ammunition inside of the chamber and (13) rounds of ammunition
inside of the magazine (each magazine collected had a 17 round capacity). A magazine pouch
containing two additional magazines (each loaded with 17 rounds of ammunition) was attached to
Parker’s load bearing outer carrier duty vest.
Photographs were taken of a Taser, Model 7, serial number with a cartridge inserted
which was inside of a pouch on the right side of Parker’s load bearing outer carrier duty vest. There
was also a cartridge inserted into the base or grip area of the Taser.
Huntington Beach Detective Remington:
At about 2015 hours, FS photographed Detective Remington as he appeared at the time of
the shooting (dressed in police uniform) inside of the Hunting Beach Detective Conference Room.
FS also inventoried and collected the following property from Remington:
On Remington’s duty belt was a Glock, Model 19, 9MM handgun, serial number The
handgun was loaded with one round of ammunition inside of the chamber and (17) rounds of
ammunition inside of the magazine (each magazine had a 17 round capacity). Attached to the
handgun was a Surefire, X300 flashlight, serial number and a Trijicon SRO sight / optics
with an unknown serial number. A magazine pouch containing two additional magazines (one
contained 12 rounds of ammunition and the other contained 17 rounds of ammunition) was also
attached to Remington’s duty belt.
Photographs were taken of a Taser, Model 7, serial number with a cartridge inserted
which was inside of a pouch on Remington’s load bearing outer carrier duty vest. There was also a
cartridge inserted into the base or grip area of the Taser.
Huntington Beach Officer Velasco:
At about 2045 hours, FS photographed Officer Velasco as he appeared at the time of the
shooting (dressed in police uniform) inside of the Hunting Beach Detective Conference Room. FS
also inventoried and collected the following property from Velasco:

ammunition inside of the magazine (magazine had a 17 round capacity). Attached to the handgun
was a Surefire, X300 flashlight with an unknown serial number. A magazine pouch containing three
additional magazines (two contained 17 rounds of ammunition with a 17 round capacity, one
contained 18 rounds of ammunition but magazine showed a 17 round capacity) was also attached to
Velasco’s duty belt.
Photographs were taken of a Taser, Model 7, serial number with a cartridge inserted
which was attached to the left side of Velasco’s duty belt inside of a holster. There was also a
cartridge inserted into the base or grip area of the Taser.
Huntington Beach Officer Simpson:
At about 2120 hours, FS photographed Officer Simpson as he appeared at the time of the
shooting (dressed in police uniform) inside of the Hunting Beach Detective Conference Room. FS
also inventoried and collected the following property from Simpson:
On Simpson’s duty belt was a Glock, Model 17, 9MM handgun, serial number The
handgun was loaded with one round of ammunition inside of the chamber and 16 rounds of
ammunition inside of a magazine (each magazine had a 17 round capacity and each had an
extender which increased the capacity to 19 rounds). Attached to the handgun was a Surefire, X300
flashlight, serial number and a Trijicon SRO sight / optics with an unknown serial number.
A magazine pouch containing two additional magazines (each loaded with 19 rounds of ammunition)
was also attached to Simpson’s duty belt.
Photographs were taken of a Taser, Model 7, serial number with a cartridge inserted
which was attached to the left side of Simpson’s duty belt inside of a holster. There was also a
cartridge inserted into the base or grip area of the Taser.
Huntington Beach Officer Koehler:
At about 2330 hours, FS photographed Officer Koehler as he appeared at the time of the
shooting (dressed in police uniform) inside of the Hunting Beach Detective Conference Room. FS
also inventoried and collected the following property from Koehler:
On Koehler’s duty belt was a Glock, Model 17, 9MM handgun, serial number The
handgun was loaded with one round of ammunition inside of the chamber and 12 rounds of
ammunition inside of the magazine (each magazine had a 17 round capacity). Attached to the
handgun was a Surefire, X300 flashlight, serial number A magazine pouch containing two
additional magazines (each loaded with 17 rounds of ammunition) was attached to Koehler’s load
bearing duty vest.
Photographs were taken of a Taser, Model 7, serial number with a cartridge inserted
which was attached to the middle of Koehler’s load bearing duty vest, inside of a holster. There was
also a cartridge inserted into the base or grip area of the Taser.

radio traffic into the Orange County Sheriff’s Evidence Lockers located in the City of Stanton.
